Michael R. “5-0” Fugate

Michael R. “5-0” Fugate, 57, of Madisonville, died Friday, April 4, 2014 at his residence.

He is survived by his wife of 36-1/2 years, Regina; one daughter, Tiffany Fugate Powell of Madisonville; five sisters, Linda Puckett, Dora Ipock and Teresa Perkins all of Providence, Adele Pryor of White Plains and Angel Haynie of Madisonville; several grandchildren, nieces and nephews.

Services were conducted at 11 a.m. Wednesday, April 9, 2014 at Barnett-Strother Funeral Home in Madisonville with the Rev. Shalmon Radford officiating.   

Burial will be in White Oak Cemetery near Clay.

The Madisonville Police Department will be providing the Honor Guard.
